Website Privacy Policy
Evolve DPT, owned and operated by Gaspar Physical Therapy
Evolve DPT (“Evolve,” “we,” “us,” or “our”) is owned and operated by Gaspar Physical Therapy. Evolve manages the website at evolve-dpt.com (the “Site”) and its website inquiries separately from the Gaspar Physical Therapy website. This policy explains what information the Site collects, how we use and disclose it, and the choices available to visitors. Patient intake and records are handled through a separate electronic medical record system that is not linked to this Site. Patients receive a separate Notice of Privacy Practices for their clinical information.
Information We Collect
Information you provide. When you submit our contact form, we collect your name, email address, phone number, and the short message you choose to enter. The form asks you not to include medical details or other sensitive information, but a free-text message may contain information you decide to provide. The form sends your submission to info@evolve-dpt.com by email. Access to this inbox is limited to authorized Evolve personnel. Website inquiries are not sent to Gaspar’s separate website or automatically entered into the patient record system. The Site does not retain a copy of the submission in its WordPress database, and the optional WPForms Lite Connect cloud backup is disabled. The site’s daily backups do not contain form submissions. If you contact us by email or phone, we receive the information you choose to provide through those channels.
Information collected when you use the Site. Our hosting and security providers may process technical information such as your IP address, browser information, requested pages, and security events to deliver and protect the Site. The Site stores a first-party cookie to remember your cookie choice for 12 months. Cloudflare may also use a short-lived security cookie for bot protection.
Unless you opt out or your browser sends a Global Privacy Control signal, Google Analytics 4 uses first-party cookies to measure Site use. These cookies contain a browser identifier that helps distinguish visitors and sessions. Analytics also collects technical information such as browser and device information and approximate location. We receive reports about pages viewed, scroll depth, traffic sources, clicks on our phone number, email address, and partner links, whether visitors viewed the pricing section on our Services page, and whether the contact form was started or submitted. We configure these events so that form contents, names, contact details, and health information are not sent to Google Analytics. Google Signals, advertising personalization, and remarketing are off. Google Analytics user-level and event-level data are retained for 14 months; aggregated reports may remain available longer. Its first-party cookies (_ga and _ga_*) can last up to two years. Google states that GA4 does not log or store IP addresses, although a connection to Google necessarily involves technical network information.
Public reviews. We display reviews that people have publicly posted to Google, including their public Google name, star rating, review text, and date. Our server obtains and caches this content, then serves it from our Site; displaying those details does not cause your browser to contact Google. Unless you opt out or your browser sends a Global Privacy Control signal, the reviewer’s public profile photo may load from Google’s servers. If you opt out, review cards show the reviewer’s initial instead. Loading a Google-hosted photo creates a connection to Google, which may receive technical information about your visit.
How We Use Information
We use the information described above to respond to inquiries, operate and secure the Site, understand Site performance through analytics unless visitors opt out, and display public reviews. We do not use the Site for advertising pixels, remarketing, call tracking, session recording, or heatmaps. The Site does not offer online booking, a patient portal, payments, or text messaging.
Cookies and Your Choices
The cookie banner provides a notice with Got it and Opt out controls. Google Analytics and Google-hosted reviewer photos may load when you visit the Site unless you have opted out or your browser sends a Global Privacy Control signal. Selecting Got it dismisses the notice; you do not need to select it for analytics to run. Selecting Opt out disables analytics and Google-hosted reviewer photos. A browser’s Global Privacy Control signal automatically opts you out of both features before they load.
You can change your choice at any time using Cookie preferences in the footer of every page. Opting out removes the Google Analytics cookies set by our Site and reloads the page so further analytics collection stops and Google-hosted reviewer photos do not load. You can enable these features again through Cookie preferences, unless your browser sends a Global Privacy Control signal. Your choice is remembered for 12 months unless you clear browser cookies sooner. A previously saved Decline choice is treated as an opt-out when we introduce the updated notice. Opting out stops future collection; it does not automatically delete information already received by Google. Some cookies needed to remember your choice or protect the Site may operate regardless of your analytics choice.
Google Maps embeds on our About and Contact pages are initially replaced by a placeholder. A map loads only if you select Show map, even if you have opted out of analytics. Google may then receive information about your visit and set its own cookies under its policies. The map drawn directly on our homepage does not request content from Google or another map provider when you view it. Directions links and links to full reviews open Google only when you choose to follow them.
You can also manage cookies in your browser settings. Blocking all cookies may prevent the Site from remembering your choice. We do not use the Site to track your activity across unrelated websites. Google may collect information under its own policies when you explicitly load a map or open a Google link, or when Google-hosted reviewer photos load while you have not opted out.

Who Receives Information
Evolve handles website inquiries separately from Gaspar Physical Therapy’s website and patient record system. We use service providers to operate the Site and handle information on our behalf, including Bluehost for hosting, Cloudflare for security and network delivery, BlogVault for off-site website backups, and the email services used to deliver and receive contact submissions. Google receives analytics information unless you have opted out or your browser sends a Global Privacy Control signal. Google may also receive technical information when Google-hosted reviewer photos load under the same preference, or when you choose to show an embedded map. We view Google Analytics reports through Looker Studio, which does not add a separate collection tool to the Site.
We may also disclose information when required by law or to protect the security or rights of visitors and our practice. We do not sell website visitors’ personal information. We do not use the Site to share visitor information for cross-context behavioral advertising.
Retention and Security
We keep inquiry emails for as long as reasonably needed to respond and manage the inquiry, subject to applicable retention requirements. Technical logs and website backups are retained according to our operational schedules and those of our providers. Google Analytics user-level and event-level data are retained for 14 months; aggregated reports may remain available longer. The cookie durations described above may differ from the data retention period.
We use administrative and technical safeguards appropriate to the information and the Site. No internet transmission or storage system can be guaranteed completely secure. Please avoid sending sensitive medical information through the contact form or ordinary email; contact the clinic for an appropriate way to communicate about your care.
Other Websites and Health Information
The Site links to third-party websites, including Google Maps, Gaspar Physical Therapy, and San Diego Orthobiologics Medical Group. Their privacy practices apply when you visit those sites. The website contact form is for general inquiries and is not a patient portal. Clinical intake, treatment, and patient records are handled separately through the practice’s electronic medical record system. If information you provide in a website inquiry is protected health information, our obligations under applicable health privacy law and our Notice of Privacy Practices may also apply.
Your Questions and Requests
To ask about information submitted through the Site, request a correction, or raise a privacy concern, email info@evolve-dpt.com. We will respond in accordance with applicable law. Requests concerning clinical records should be directed through the process described in our Notice of Privacy Practices.
Changes to This Policy
We may update this policy when our Site or practices change. We will post the revised policy here with a new effective date. When our cookie practices change, we may show an updated notice or ask visitors to review their preferences. Showing an updated notice does not cancel a previously saved opt-out.
