2. No Doctor-Patient Relationship

Use of this Site, or the transmission of information through contact forms, does not create a doctor-patient or therapist-patient relationship between you and Evolve DPT. A formal relationship is only established after you have been seen for an initial evaluation and have completed and signed our clinical consent and intake forms.

All content on this Site is the property of Evolve DPT and is protected by copyright, trademark, and other intellectual property laws. You may view, download, and print content for your personal, non-commercial use only. Any other use, including reproduction or distribution, is strictly prohibited without our prior written consent.

This Site may contain links to external websites, including those of our parent organization Gaspar Physical Therapy, San Diego Orthobiologics Medical Group, and other partners or resources. 6. Your Right to a Good Faith Estimate

Evolve DPT provides self-pay physical therapy services. Under the No Surprises Act, you have the right to receive a Good Faith Estimate of the expected charges for any non-emergency items or services before they are provided.

Your provider is required to give you a Good Faith Estimate in writing at least one business day before your scheduled service. You may also request a Good Faith Estimate at any time before scheduling.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute that bill.
